# booking-form

> Books the slot picked in its booking-calendar without a cart (an appointment): the booking, with the form's name, email, phone and note fields and the logged-in customer, is saved in the store's Bookings (Shopify admin → Content → Metaobjects) and shown on the product's Smootify booking block. Webflow's success and error states show the answer; a slot that filled up meanwhile is the form error and the calendar shows its places again.

## Where it can go

- **Anywhere** on the page.
- **Must contain** [`booking-calendar`](https://www.smootify.io/docs/reference/elements/booking-calendar), `form`.
- **Out of place,** Smootify removes it from the page: also on a site whose licence has no “Booking”; logs an error without a form.

## What it requires

- The **server plan**: on other plans Smootify removes the element from the page.
- The **Booking** extension (server plan, the extension activated in the Smootify dashboard, the licensed domain). Without it, its markup is removed from the page.

## Fields in the form

Each field’s **Name** setting in Webflow tells Smootify what the field holds. It is a setting of the field, not a custom attribute.

| Name | Field | What it does |
|---|---|---|
| `name`, `email`, `phone`, `note` (any case: Webflow’s `Name`, `Email` work) | Text field, Text field, Type Email, tel, Text area | Written on the Booking entry the submit creates in the store, trimmed; an empty value is not sent. |
| any other name | — | Not sent: only these four reach the store. |

## Attributes inside it

| Group | Attribute name | Values |
|---|---|---|
| [Booking form fields](https://www.smootify.io/docs/reference/attributes/booking-form) | `name` | 4 |
